You may request a copy of the PHI we maintain about you. We may charge a reasonable fee for copies.

If you believe your information is incorrect or incomplete, you may request that we amend it.

You may request a list of disclosures of your PHI made by us (excluding those for treatment, payment, or operations).

You may request limits on how we use or disclose your PHI. We will consider your request but are not required to agree.

You may ask us to contact you in a specific way (e.g., only by mail or at a certain phone number).

You may request a paper or digital copy of this Notice at any time.

We are required by law to:

• Maintain the privacy and security of your PHI
   
• Provide you with this Notice of our legal duties and privacy practices
   
• Abide by the terms of this Notice currently in effect
   
• Notify you promptly if a breach occurs that may have compromised your information
